Activity Waiver-
For the soccer tournament on June 13th, in consideration of being allowed to participate in the activity, I agree with Sparrow Baptist Church, 2 Jane St. Toronto, ON M6S 4W3 to the following.
Legal Release-I am either of lawful age to participate or have received consent by signature below from my parents, to participate in this acitivty. I the participant release and forever discharge Sparrow Baptist Church, its owners, officers, directors, employees, and volunteers from all manner of actioins, casue of action, claims or demands for or by reason of any injury to person or peroperty, including injury resulting in death of the participant, which has been or may be sustained as a consequence of the Participants participation in the Acitivty and not withstanding that such damage, loss or injurty may have been caused solely by the negligence of Sparrow Baptist Church.
Fittness-I confirm that I the participant do not have any physical limitation, medical ailments or physical or mental disabilities that would prevent me from participating.
Allowed Behavior- I agree as the participant, or legal guardian of a participant that there will be no alcohol, canabis, or smoking allowed on premises of the tournament and that no profane language will be allowed.
