We ask that your child be kind to other children and respectful to counselors while in attendance. If they do not follow these instructions you will be called and asked to pick them up promptly.

Your child will be constantly supervised and proper safety precautions will be used

Release and Indemnity

I, the child’s parent, hereby grant my child

permission to participate in the Sparrow Baptist Treasure Hunt Day Camp.

The participant and I release the corporation, Sparrow Baptist Church, and it’s volunteers, from all claims, demands and liabilities, actions or causes of action in anyway arising out of the above activity including the participants participation in the activities of the camp and use of any facilities of the church.

Without restriction the generality of the foregoing, the Participants and I do hereby release and forever discharge the Corporation, its volunteers and employees from all claims, demands, liabilities, actions or causes of action for personal injury, or death or damage to any property the participant possesses which may be sustained during the activities of the camp, except as may arise from acts of negligence by Sparrow Baptist church to follow their camp and church policy.

The participants and I furthermore agree to defend, indemnify and save harmless the Corporation, its employees and volunteers all claims, demands of liabilities which may be made by third parties in any way referable to the Day Camp of Sparrow Baptist Church.

This Release shall be binding by the participant and Parent and their family members.

Upon signing this form, permission is given to Sparrow Baptist Church to seek Medical Help for your child in case of emergency. Any cost incurred will be the responsibility of the parent. Also you are agreeing to the behavioral policy and the release waiver.

I hereby, give my permission to Sparrow Baptist Church Day Camp the right and permission to produce, publish and use my child’s photo. This will be used as promotional material for the Day Camp and Sparrow Baptist Church only.
